Probably Stormwind from WoW... everything was sectionalized, labeled on map, easy to get to, plus it was one of the few cities that had lifelike npcs that walked around and mingled alot.
Or Rivendell of from LOTRO, possible the most beautiful city I've seen in a game yet, but I didn't spend too much time inside so I don't know about its ease of access, I was too busy on the cliff overlooking it pissing my pants and picking up my jaw.
I can only speak on the games I have played but I will have to say the city of Aden in Lineage 2. I love the music and the design. Also it has a very beutiful Castle outside in to the North of it.
Stormwind. I love the epic, rolling music as you make your way through the front gates. The fact that it's always brimming with players helps make it feel 'alive', too.

Played so far: 9Dragons, AO, AC, AC2, CoX, DAoC, DF, DnL, DR, DDO, Ent, EvE, EQ, EQ2, FoMK, FFO, Fury, GW, HG:L, HZ, L1, L2, M59, MU, NC1, NC2, PS, PT, R:O, RF:O, RYL, Ryzom, SL, SB, SW:G, TR, TCoS, MX:O, UO, VG, WAR, WoW... It all sucked.
Best city I can't put my finger on but best city atmosphere would have to go to any major port in Pirates of the burning sea, ...NPC fights breaking out....soldiers chasing criminals around the narrow streets....bands of drunk pirates walking down singing a yo hoho......street urchins thieving from shop sellers stalls...frisky drunken sea dogs propositioning passing maidens and getting a well earned slap across the face.
I always liked stormwind for its efficient design but overall I have to say I really liked Droknar's Forge (guild wars), it just felt so epic especially the first time you discovered it.

I'm still fond of Britannia from ultima online and how the community developed around the space it offered. Brit library where real live players gathered to sell their crafts face to face with players was something to experience. The graveyard was always fun. It was always so alive.
Qeynos in everquest was a fabulous city to grow up in for a 3d mmo. Someday I hope fippy's raid on the city gates is succesful.
There are a number of other great cities worth honorable metion.
Ill agree with stormwind right from the start its just amazing, when you see the statues of the dead or missing heroes then you have the a train there that takes you to another city, whats not to love about SW

i always had a soft spot for Mos Eisley in SWG, city felt like it planted you right into the movie.
Ignore all other posters :-), these are medieval *towns*.
I say the best *city* is the one of CoH, withouth the border walls.
I would say Stormwind from WoW or Rivendell from LotRO.
Kelethin in EQ1 was my favorite, something about a city in the trees was badass, but than again it was my first MMO city experience.

I'll begrudgingly say WoW. The sheer scale and design blowed you away the first time you enter into say Ironforge or Orgrimmar.
