What would yours contain? Mine would contain:
A Medieval Fantasy theme
A vast open world with instanced dungeons
A challenging but not ridiculous crafting system
Unique quests that actually tell the lore of the land
Some grinding
P2P
No Cash Shops
Hotkey Bars
Advantages to working hard for your items
Elaborate dungeons that hold the rarest loots
Select advantages per race and class
No Fast Travel unless you have visited the location first and only as some sort of flying creature, no horses like LOTRO.
Farming, as in planting a seed and waiting for it to grow
The ability to buy land
Open world PvP
Guild Towns
Guild Town Sieges
The ability to conquer and rule.
A working economy
Random events such as a group of trolls attacking a city or village
The ability to travel alone from time to time
The ability to experiment with raw materials, fuels and temperatures to create new materials and what not.
Warring Factions
Separate worlds for those of us who choose evil over good or vice versa. I.E. having to sneak through sewers and cracks in the walls as an "evil" being in the cities.

^^ what he said...pretty close to mine as well. Although I would like PVP/PVE missions/quests like Global Agendas Double Agent.
1. games for adults. i don't talk about nude or ultra-violent games, i talk about a more challeging gameplay. many of the todays 30+ generation have still interest in mmorpg's. well, it would be very interesting to see a realistic simulated gameworld, without any influences from disney world or pokemon. i really hate pastel colors!
2. there are many good settings. choose maybe the postapocalytic, scifi, medival or a mix of all. just don't forget: you develop a game for matures.
3. open, open, open. we do not like grind or any other dull instances. if someone kills boss X, boss X, should be dead forever. the devs... or the AI... have to bring in new stuff.
4. community involvement. let em create content for you or let em vote about your own created content..
Well, my whole reason for trying mmo's was that I was, at the time, on a big morrowind/neverwinter nights kick.
I thought that mmo's were like those except with other people.

1. With a tortage-like tutorial/starting area(s) before sending you out in the world.
2. Also, a diplomacy function like Vanguard's that would also include it for guild-level diplomacy (leads into conquering or building alliances to rule territories).
3. No quest markers.
4. Indirect quests as well as direct quests. Indirect quest = local NPCs complaining about goblin raiders, you (and your friends?) go and eliminate the raiders. The NPC village would grant rep (good for diplomacy), some gold maybe a "sacred" item.
5. Slow and intelligent mob respawns (requires a huge world).
6. SWG-like crafting, harvesting with item durability that wears out with use can only be repaired to a portion of its full durability that lessens with each repair.

1. randomized resource node locations and trade caravans needed to move "heavy" resources around
2. limited inventory looting ( non weapon / armor )
3. twist pvp ideas ( ie. Necromancer slowly gaining power through class specific quests/ceremonials and be able to wield large undead armies but with every gain of power u would emit more easily trackable aura so Templars/Paladins can find u and reset the power gains )
4.player specific crafting "recipes" so exact same recipe will wield different result from other players thus forcing experiments.
5.item stats to be more closer to practical rather than buffs for player HP/DMG ( ie. better quality armor has more % to shatter arrow /deflect a hit ... better bow = more range / penetration ) with movement etc penalties the heavier stuff u wear.
6.dying ingame would teleport you to your pre-designated player run village/NPC city and add a rez timer if you die too close to them to prevent spawn point running.
7.special prices for killing Bots / gold farmers
8.multi NPC faction central hub where u can list for ex. to Army and get some benefits / supply of decent armor but need to do Patrols/fight wars/catch criminals to earn your pay.
9.gambling
10."jail sentences" for smuggling , murder + multitude of other criminal activities happening in NPC governed areas ( this !!! i just love being Thief :P )
could go on and on but those are just the top 10 that comes into mind besides all the stuff OP and others allready posted.

I've seen players playing in the same spot for ages who were accused of being bots. They didn't look like bots to me but becasue they were in the same spot "suddenly" they were bots.

naturally it would be to developer to reward the players after they have verified / banned the account for botting ... but yeah all in all it might be abit too much hassle . i added that mostly as a joke but hey its still an idea hehe
Oddly, I don't have a list for the "perfect" mmo.
What I want changes for the genre.
What I want, or expect, from a super hero mmo is vastly different from a
space faring mmo or a high fantasy mmo.
If a game is coming out and sounds entertaining, I wait for it to come out and
read the reviews posters make. Did the company deliver on what they claimed?
If they did not deliver on everything they said they would, then I have to ask:
Are they continuing to try? Is there potential for positive change in the direction
the community asks for?

i often found that i was too lazy to report obvious bots in the games ive played ... so u could take it as motivating the playerbase to take part in the fight against RMT. i bet i have been reported too because of my habit to stay at one spot and farm mobs for 8 hours straight :P
edit: the reward doesnt even have to be anything more than a title for ex. >Bots Hate Me< .. that would make me happy
I have such a long e-mail list of all the responses from NC Soft whenever I reported a gold advertiser in Aion that i had to make a separate folder.
They used to be pretty good at removing them. At least in a few hours. So good in fact that once I had logged in about 6 or 7 hours after the fact and saw someone I reported. I almost wrote them again to scold them.
I think my perfect mmo would have gm's who patrolled the game and made sure that areas were clear.

A completed version of Infinity: The Quest for Earth, with next-gen graphics and the ability to get out of your ship and have an avatar, with F/TPS-style ground combat.
OR
A realistic world completely uninhabited by NPCs or NPS cities or anything. Every play starts with nothing at all and you have to work to build a society and civilisation from scratch, which in the later stages of a server would reach as far as modern day society and even into the future.

Take EVE online and D&D and smash them together with a bioware story line and the polish of WoW. And make it in an apocalyptic/horror setting with zombies....lots of zombie.
