My darkfall character has 100 skill slots and even a few more character functions outside of that and I need to be able to access any of them pretty muuch without looking and without moving the ball of my left hand much at all.

Keyboard and mouse and gamepad. I switch back and forth between them depending on the game, and sometimes depending on what I'm doing even within a game.
I don't think there is enough distinction between a "gaming" keyboard and a normal keyboard or mice to warrant two separate entries. Regardless, keyboard and mouse for everything except maybe racing games and even then I'd rather have a steering wheel.

It depends on the keyboard. Mine has a secondary keypad that place all the important keys around my hand. I can press 0 just as easily as 1. This means that I do not need to macro things or use extra key combinations.
