It's exactly the same as Diablo 2, if you played that.
Basically, think of it as a game with 16 Acts, Normal 1-4 Nightmare 1-4 Hell 1-4 Inferno 1-4, the monsters at the start of each difficulty level are of a level just beyond the monsters at the end of the last act of the previous difficulty level.
Difficulty is really just a different level tier. Normal is 1-30, Nightmare 31-50, Hell 51-60, and Inferno 60+ (gear progression only). So to start a new class on say nightmare that would be like having a instant level 30 button. It would defeat the point of the game which is level, and gear progression.
