I've tried doing the guild leader / officer thing a few times. It zaps all the fun out of the game. I feel like I'm playing "General Hospital Online" dealing with members petty squabbles. Also this misguided notion that I'm supposed to make the game fun for them.

I laughed heartily at this. Then I realized this was me, too!
I seem to get into groups where everyone already knows what is happening while I get left behind fighting MOBs they skipped.
I am a follower, but would like a group where we figure things out as a group. Overbearing leaders put me off. I avoid this type of player whenever possible. If given a choice between "my way or the highway", I'll choose the highway every time.

Some good questions. I'll try to answer as well as I can
1) I don't know. It seems to me that my "drive" factors into it more. If I have to think up the reasons why I am doing something, it is not quite as enjoyable for me.
2) Someone who has good ideas and tactics, yet is open to suggestions.
3) Someone who will add to the group with their ideas and notions.
4) Most definitely. I have started zero groups in my 12 years of playing MMOs.
5) Finding a good leader is just getting to know them in the game. Grouping with them or being in a Guild with them. Time i the only way for me to tell with a good leader.

Personally the only modern MMORPG trend that annoys me is the idea that MMOs need to be designed in a way to attract people who don't actually like MMOs. Which to me makes about as much sense as someone trying to figure out a way to get vegetarians to eat at their steakhouse. - FARGIN_WAR
For years, I usually embraced the leader role. It was mostly because I always went by the rule of "if you want something done right, do it yourself." However, lately I've been relegating myself too something I like to call "diligent support." I pay attention and follow orders, which is what DPS is supposed to do. I think I've hung up my Tanking shoes for good. Gaming has become much more enjoyable since I did.

I'm as comfortable being a follower as I'm being a leader. Really depends on the situation and if there is a better/equal/passable leader around, and if I'm interested enough to be envolved enough to become a leader.
I prefer be the leader's advisor, "best friend" or "favorite" than the leader itself. I like to have a relatively "weak" leader who i can "manipulate". In that way, i can enjoy almost all the benefits of leadership without almost none of its burdens and disadvantages.
What i am talking here occurs in a pvp sandbox context. Maybe that approach doesn't make much sense in a pve themepark context.

Mostly I'm a bit of a lone player, though.
