First off I have to admit that the game is a lot of fun. The graphics are nice the music and the overall imersion are unpaired.
The sandboxy elements for classes customization are also very fun and leave some space for theory crafters.
On the other hand my game crashes a lot especially in more populated Cyrodiil. My computer is a monster and I can run any games on the market on the very high settings.
Sometimes my weapon switch bar gets locked and I need to relog.
Sometimes the game just freeze.
Sometimes I cant open doors or talk to NPCs and I need to dodge roll and switch weapon to work around that bug.
But the most anoying thing is the game Responsiveness!!!
I dont understand why sometimes even with VERY low ping I press a button and nothing happens. I press it again and nothing happens. Press it again and then hold for 1 full second and release it and it FINALY happen.
I can go for a bit and everything seems to be polished until the next clunky unresponsive moment.
You click attack and special ability to weave in normal attack in between abilities and sometimes the light attack just wont happen even with perfect timing.

Its realy sad because the game has a lot to offer and is realy fun.
I just cant stop thinking that GW2 combat and UI is so much better. On the other hand GW2 is a lot less immersive and feel more instanced.

I played at launch, got a Templar through a few VR levels, then got a bit fed up with it.
Recently returned as it's free for me, started another class and been questing. It does feel less responsive than I remember, and I've also had a couple of game crashes. I am seeing delays in skill abilities, doesn't seem any pattern to this.
Combat feels a bit, I dunno.....unattached.... at the moment.
Mind you, I have come from leveling a toon in Neverwinter, which has some of the best combat mechanics around of the current crop.
Search for the IP address of the server and do a ping tracert,perhaps you have severe packet loss which can happen even at 40 ping.
I have actually noticed some very odd anomalies with pings over the last few years,i tend to get more lag from extreme low pings when compared to mediocre pings.
I would also not rule out a ton of malware or even a virus.This site is a prime example of anomalies in the network,some days it loads fine some days it just cycles forever and takes like 20 secs to load a page.
Never forget 3 mile Island and never trust a government official or company spokesman.
My machine is extremely good (not a monster) and I absolutely don't have these problems.
I'm not saying you are making this up but I suspect that "those people with monster machines" might have machines that fall outside the norm and the game has issues with certain things on your side.

Everything is smooth and responsive outside of Cyrodiil, which occasionally has real lag issues I remember standing there pressing the door button for over a minute trying to get through a keep wall. My guess is they weren't quite ready for the influx of players or this was some kind of bug with 1.6.
Either way since I have no issues outside of Cyrodiil I assume it's nothing to do with my connection or some fundamental problem with the game.

A long time ago I installed something called Leatrix this might have to do with my packet lost?
I read a lot of people are having the same issues so I am not the only one with this problem.
when I entrr houses the whole world becomes white for a bit and all I see are objects and npcs for a couple seconds. I didnt have that problem before 1.6
Dont worry, game has been unresponsive since i started playing it (new year) so i might claim since launch. Theres no pattern to it, sometimes it just refuses to react. Its not even only combat, sometime you have to wait 5+ seconds just to start using gathering node/doors.
Even if the whole map was empty (like veteran levels)
But i guess its also a matter of perspective, if you come from games like GW2 and wow it will be unresponsive, if you come from games like SWTOR or FF14 it will be refreshing.
And i guess its most noticable in AvA, PvE is super easy even in veteran levels, but in AvA you cannot really have a luxury of waiting for game to make up its mind will it fire the ability/attack or not.
I ran into an issue that can reproduce many of these issues. without a frame rate limiter, ESO was causing my Nvidia GTX470 to overheat. I had all kinds of weird things happening. Can you view your graphics card temp while experiencing these issues?
Though I don't have any freezing or crashing issues like the OP, the only problem I've had for the longest time is that when hitting 'E' to interact with something like a door or an NPC, the button is white..I click on it..and instead of interacting it turns Grey..I wait a bit..nothing happens. I move back, try to interact...same thing. Its very annoying, but I chalk it up to just one of those annoying MMO things that I hope stops one day :-). Still love the game.
I thought it was unique to my gaming laptop that had dual 580Ms, but now I have a Falcon Northwest Tiki with a Titan X and Devils Canyon CPU so I know its not my hardware, not to mention my internet is 110mbps and I have latency only between 80-120....and an Acer 1440P Gsync 144hz IPS monitor with just about zero lag time.

Yup, thats the thing. And its not just doors, its any interactible, and i would guess when in combat it manifests as not executing attack/skill/weapon swap. And it has no connection population, latency, ive been playing for 4 months now and it happened even in deserted veteran areas.
I have not been having any of the issues you describe since I started playing again. Which was a few months before 1.6.
My computer is not a monster but very decent. Run the game on custom setting with everything on high or > with shadows on medium.
I have had two game crashes tho. But only two in the past 3 months or so. Nothing major.
None of the other issues you have described.
I don't think the "E" interact thing is lag - there's some distance-related funkyness that actually got a bit worse in 1.4 or 1.5... somewhere around there, It's annoying but it seems to me it works better if you're at the far limit instead of really close.
It happens often enough that I expect it to happen now.
OTOH, I have no issues with combat responsiveness nor "city lag" of any sort.

I get this also from time to time. Not as bad as it use to be when the game first started but it's still there. I would call it rare for me and hasn't caused me to die during combat.
