I've seen it hundreds of times... Epic'ed out characters - dressed in black and looking badass... Until I look up to read their names... BananaNiplz, SkillCosby, SmellyGirl, etc. looks so stupid.

Or you could do like in the real world and allow duplicate names. Databases have had to deal with that for decades.

That sounds like a no-win situation to me.
Stupid people uses stupid names. In some cases however (certain games) you have a rather limited numbers of letters and everything that sounds exceptable is already used but sensible players still stay away from nimes like L33T, nobkiller and variations of a certain elvish name.
Funny thing is that in games with RP servers it seems like peoople with stupid names are more frequent on the RPG server then on the regular servers.
Agreed, it's probably one of my biggest peeves as far as online gaming goes. Not long ago I played an MMO with a friend who I knew gamed yet we never played the same games at the same time. WHen I saw how he named his toons (which i won't share here) it will probably stay that way, him : "hey man you gonna log in?" me: Nah, I'm playing something else now.." <-- default answer...

The real question is how do friends lists work, tell/reply systems, etc... Unless you use account name for that, yet then what is the point in names if we can't identify or communicate under them? I don't want to send tells to Alias@567.net.. that would be even worse than the issue the op is pointing out.

I like seeing fun or clever names. Shows that they don't take it too seriously. And maybe that they are fun people. Even if the game is pretty heavy into immersion through character (like lotro). In my book you get a pass on 4th wall breaking if your name makes me grin or chuckle. There are those names that are totally phoned in though. Where it is clear that a person didn't even try. That said the older and more popular a game is/has been, the harder it can be to get an unique name (especially if they don't free up old names once in a while). Also, some people purposely choose names that have deceptions as to how they are actually typed. In-so-much they make it hard to be contacted and they either like it that way or they needed to use a faked character to make their name work. I will say Goku1792carrot is a little annoying. But, beyond things that 'no effort' I don't care much about it.
Now if you want a good fantasy name. A method I found useful is using parts of scientific names. Maybe from part of a pets scientific name. My name here, which I use in a lot of places, is part of one... Crotalus Oreganus Helleri (the Southern Pacific Diamond Back Rattlesnake). You can also change a letter to the name of a medication like... Luvesta, Diagra, Bialus.
The most important rule in gaming is have fun and never take yourself too serious. Fun names express a sense of humor and more then a one dimensional personality.

Your name is whatever you want it to be the account name would be in a second or third hidden slot assigned automatically that no one would see.

They don't get invited to groups either, probably because when you see the name they use, you know what to expect from them.

Say you just got destroyed by someone in pvp, doesn't it make it even worse if the guy's name is something like Catflaps or Adolfcritler?
That's why I like naming conventions in games like Ryzom. No "MeR0XX0rUSuXX0r" folks around. I find it pretty unimersive. For those who don't care for lore, or RP... Why the heck destroy immersion for others? Just for the fun of lowering the experience of other people you don't even care for? Why someone enjoys something like that is unknown to me, perhaps I'm getting old after all.
Names should roughly fit to setting and lore. Not even completely. If names are more of callsigns (I see them that way in Anarchy Online, as you can have a regular first and last name, too), I can pretty much accept a "Starkiller", in a complete fantasy game it is rather immersion breaking.
As for the adding insult to injury argument in PvP... At least I do not care for the name of the guy who downed my char and judge based on the behaviour. That's the point someone can lose respect rather quickly. Congrats for a good match: Rise in respect, someone who is a sore winner and badmouths others after winning loses respect dramatically. I guess that's ture for others, too.
