If Blizz is to make another huge game in scope of MMO it will need to have more "kid" friendly imagery than Diablo 4. While WoW subscriptions are plummeting Blizzard is making 20 million dollars a day ! with Hartstone. F2P minigames and MOBA are new kings. Blizzard dont want to make new MMO - and this is probably why Titan was scraped in first place.

Well, the add were talking about a game, not an expansion but it is surely possible that is what they meant. They could however already be working on the second expansion since long and let this be for something else.
Heck, this could actually be a CCG like Heartstone set in the Diablo IP. Those are cheap to make and in HS case it earns loads of money.
It could also be Diablo 4, knowing Blizzard we are talking a long development time for it. D3 was originally rumoured to be released in 2008 before they scrapped that version and Blizzard North with it.
They could be making a RTS game or even a skyrim like RPG in the world. It could be a Diablo MOBA.
Or it could be a MMORPG, we just don't have any clue. Actually aiming on older players might not be a bad idea, the average Wow player today is hardly 14 and the older Blizzard fans who used to play Wow are probably even older. Adult styled AAA MMOs are actually not that common, you have AoC and maybe a few Korean ones.
My point anyways is that we need more information. At this point it could be anything.
I don't believe I could play a Diablo MMO in it's current state. It would likely be WoW with a Diablo 3 skin. If the game used the original classes and art style I would be more inclined to give it a try.
I guess I mean, a game with combat like D3, ect. But with players like WoW. I guess the closest thing to it would be PoE, even though you can really only fit a few players in a party. And you see a few in town, but that isn't really an MMO imo.

Lineage was a game with combat not unlike Diablo and it was made in 2000 so I don't really see those technical limitations. The problem is more to make it fun, Diablos combat tend to be about being swarmed by mobs without much group dynamics while drinking pots with a speed that would shock the town drunk.
I don't think you could pull off a MMO with exactly the same mechanics as Diablo (any of the 3), you need to add a bit of group dynamics into it as well. You also need a pretty large world, probably with instanced dungeons. But there is no technical reasons you can't do that, you just need to figure out how you make the group playing more fun.

You kinda have the same limitations in any MMO, you get plenty of opponents against you in a game like GW2 as well and yet it works excellent there. Sure, we ain't talking about the absolutley latest FPS graphics but I don't think GW2 have worse graphics than D3.
OP was mainly refering to the new Lineage game and if the Koreans can do it there then I don't see why Blizzard should have more issues with it.
If everyone on a GW2 server would for some reason stand in a single point as have happened during living story events the game performs aweful on a standard machine (happened to me during the Southsun cove end event), my latest build have however performed excellent the last year since I got it and I never gotten below 35 FPS during anything.
So I get what you mean but while it do take a good computer and server it is not that hard to solve.
Imo, if they make some kind of MMO from Diablo, it is likely to be something more like Mobas and less like WoW.
That's like saying "if they make some sort of cake from Diablo, it's likely to be something more like a spaghetti bolognese and less like a chocolate cake."
D3 as an MMO would be brilliant; the lore is in place, the main story has been concluded, this would be a great space for players to explore and clean-up the leftover demons.
Marvel Heroes is IMO the best option out there. Of course, developed and managed by the creator of Diablo, Devid Brevik. Hell its even more fun than Diablo 3 for me.
Path of Exile is another good option, but is heavily instanced and doesnt compare with MH IMO.
Path of Exile is a great game but should NEVER be recommended to someone looking for an MMO. If they wanted a regular multiplayer APRG then sure. But when someone asks for an MMO its best to actually share MMO's with them.

and actually lineage eternal is more than both, if they still have all the original ideas in it (those they have showned us like 2 years ago)
