I was one of those guys who just got one character to max and just only played that one.
Of course that shaman was top notch when I had time to get him there, becoming the 2nd best enhancement shaman on my server during first half of WOTLK. Of course I also traded items back and forth between factions using 2 accounts and had an assortment of best dressed bank alts (my drenai bank alt wore tauren level 1 cloths
).
Anyways, I had recently installed an addon that would autoreply emotes. Being a tauren named ravtehcow, I had it setup to /mmo back to anyone who /moo'ed to me (and more).
My "friend" and guild tank that I would run paid-instances with (as in other people would pay us to carry them through 5 mans and Kara and even ZA in TBC) decided to have a little fun and installed the same addon.
He had it setup to /moo back with no delay. He then typed /moo to me.
I was suspended for 3 hours. "Reason: Ravtehcow typed /moo 6000 times in 30 seconds."
My tauren was banned for moo'ing. Yes, that's right- a cow banned for moo'ing.
Of course in Wow I was suspended for alot of things.
Standing on the mail box on a kodo with stacked size buffs (that happened alot).
(Before the nerf)Running in the bank against the wall with my 3-man mammoth mount to shake everyones screen in the bank.
Relogging to double stack the troll witchdoctor (in the troll starting area, requires exalted) size buff, then stacking more size buffs and then bloodlusting to become taller than Orgrimar itself.

I was using a stormherald at level 70 in the 5 man heroic. (thats almost 10k in one swing at level 70 against a 5-man heroic boss)
Of course you could instagib just about any druid or mage in WSG with that thing.

I did get banned from the RIFT forums for talking about how it resembled WoW in one honest-to-god, completely polite post.
I got banned from my then-guild for bidding one point of DKP (good old days) on an item that I wanted before it went free-for-all. I didn't bid as much as the people actively interested but the leader was one of those actively interested and he took a lot of offense that I bid 1 DKP.

I redecorated my forum post boxes to have a nice background color and changed the borders. I also changed my avatar to this
and made it really large and made it hover over the left corner.
I also made my arch-enemy's forum post box bright pink and changed his signature to
(his name was railgun the mage)
It was some epic javascripting bro.
Don't get me started with all of the things I did in shadowbane to get banned....
If you bring it up the thread gets locked and the mod will say "Please PM so-and-so to discuss moderation".

In the Wrath<->Cataclysm transition on the PTR, flying wasn't allowed but if you flew at the dalaran portals just right and clicked them before Dalaran dismounted you, you could fly around. I did do that, but I was ready to quit or be quitted at that point anyway, not that it was anything huge.

GM and I got into a heated debate over roleplay and mechanics and he eventually brought in a lead GM that stuck me on one little square of grass in the middle of the ocean and left me there. Nobody trusted me to let me on their boats to get me off the island so I had to wait for a friend to level up and get enough to buy a boat and come get me.

That's about it.
I didn't get banned for this, but I had the top 2 tiers of the ICC tank ring.
See, I had repgrinded up to second highest tier when I saw someone was selling the best BoE tank ring in the game, so I paid him the gold for it. Equipped it.
After this, I rep-ground up to get the final ICC tank ring.
Well it turned out that he had ninja'd it somehow so the gm's took it from me and sent back the ring I had previously had equipped in that slot which was the second highest tier. So I'd run I had basically the most awesome rings ever.
I login one day, a month later, to a mail from a GM ("blah blah, we're taking your ring, don't let this happen again")

When it starts costing them money thats when they get serious.
As for me, never been banned. I don't play games to cheat. Kind of defeats the purpose.
The OP's convictions were at least for playful tomfoolery, which is never really a bad thing.
When someone exploits to get ahead in a video game that really does not matter it always boggles my mind.
