To be more specific, The Forgotten Realms.
I've been reading those books for years and years and years and feel as if I know that world like the back of my hand. When I started MUDding back in the day I wanted to code my own MUD based on the entire TFR world. Yeah, that didn't happen...
Completely personal preference here but that's what I'd like to see.

That's a hard choice, because I can think of a very good game for many of those.
You have a vast world with tons of lore and a great skill and level system in Forgotten Realms and Dragon Lance. There are also many really good fantasy worlds written down in books that would be interesting to see in a MMO.
You could also have a MMO that is Medieval in the realistic sense. Player ran Castles and if you have the skill Empires, that can be sacked and taken over by other PC's. Magic wouldn't play a part in this and neither would magic folk. Just plain old Medieval lore in the Middle Ages.
Space, which is what I would vote for, is still unexplored for the most part in the MMO genre. I love the Star Wars Universe in the KOTOR era myself, but Stargate, Star Trek, and just about any other T.V series or Movie with an in depth Universe would be great. Even a made up one with a lot of depth, several worlds, space stations and space ships, and player ran colonies would be awesome.
Earth now would be cool if done realistically as well as Earth Future. As long as it's no post apocalyptic, because I really don't like that setting or the Cyber punk settings.

For a preset theme I'd love to see something based of Magic: The Gathering. There's so many untapped fantasy races etc. in MTG that could make for some pretty deep gameplay.

I would love to see a game based on David Webbers writings of Honor Harrington (spelling). The combat system alone would draw me to another space based game. Can you imagine a battle that starts off with the two sides so far apart that closure time will be in 30-40 mins. With massive waves of missles being thrown back and forth?
This other game, just lacked that ability, 5-30sec combat, you cant enjoy. And thats even if your computer was lag free, forget about it with 56k modem and a older computer.
