WHY NOT!? It actually existed during the closed beta for Burning Crusade, not that it was great but still... I say: The Sword of a Thousand Thruths! (see the difference, I added "the")

Meat Tenderiser in Runescape. It wasn't really that powerful, but you had to complete the hardest quest in the game, which took me a few months to complete. It looked really cool, and if a newb saw you with it they were like "Omgz nice hamur!!" It was fun

Uhm, a really cool item I always wanted was in EQ1. This item was the wizard epic, "The Staff of the Four". It was released in the second expansion of the game and now there are 13 expansions currently so the item sucks big time but it used to be a really neat staff and I always wanted it.

Problem i see with these items you guys are posting, contrary to the one i posted is...
They are only upgrades, where as the Xelima sword was a tactical item. You would lure a guy out, and then equip your sword, activate it... and have some of your friends staying invis around your area, and he would be deadmeat.
There was also other items such as this in Helbreath, another item was the Medusa Sword, which paralyzed whoever got hit by it, another tactical item.
A third item was the Meridian Plate, and Meridian Shield. The plate was supposed to destroy all weapons that attacked the wielder, when it was hit, and the shield blocked all spells and melee attacks for 10-20 seconds.
These items were _UBER_ rare, the 3 years i played Helbreath, i might have seen 3-4 of each item. So yes, they are overpowered in certain(note: certain) situations.
This is what i think may Helbreath such an amazing MMORPG, and by far the best PvP MMORPG ive played.
"Great items" for me are not rare uber-weapons. More the things, that affect a whole group or even zerg.
In WoW there was a whistle (not sure now, long ago). You can make a whole zerg dance with it. Or in DAoC some artifacts, make the whole group look like animals.
Well this will seem corny as hell to most of you because it was a weapon that you couldn't actually use...
However since all my mains have been Paladin types i'll have to go with the Sword of Truth from Lord Seru.It had crazy stats for that time but alas you couldn't equip it..
See I told you it was corny...
Second place was this crazy sword I got from Plane of Water that had a freeze damage proc. Man I used it on a raid once and broke a ton of mezzes. I still haven't lived that one down..

I gotta say though, my Epic 1.0 in Everquest was more than worth the hassle.

I don't know about the greatest, but one of the coolest is that item in WoW that forces people nearby to dance.
