My account got banned when I Quit...

And I don't know why. It's been about 1 year now and it got banned after I quit, or so I think, cos I played it right up till I quit. And I don't know why its been banned.
I was also wondering if it could be ever recovered if banned under false/mistaken circumstances?

I found I was banned for a chargeback of $14.99 and just need to pay it to have my account unbanned. But the only thing is I dont remember the last 4 digits of the credit card I used to first bill the game
