You can get the campaigns separately, or now you can buy all three at once (Guild Wars Trilogy) for $50 on Amazon. Buying Nightfall separately does /not/ include Prophecies automatically. With no monthy fee, buying a box is all the money that ArenaNet gets. They're not just trying to get you to get started cheaply so that they can extract monthly fees from you.
As for which campaign to start on, it depends some on what you're looking for. If you do content for the sake of doing content and like an interesting challenge and a good storyline, I'd recommend that you play through the campaigns in the order in which they were released (Prophecies, Factions, Nightfall), in order to get the proper experience. If you do Nightfall or especially the Eye of the North expansion first, you'll get stronger faster, but it kind of ruins the earlier campaigns, especially the pve-only skills.
If you're trying to get your characters as strong as possible as quickly as possible, going to Nightfall first and then the Eye of the North is the way to do it. But if you're just looking for a grindfest, then Guild Wars might not be the right game for you.
Nightfall consists of a completely different continent, but includes all professions that comes with Prophecies as well as two of its own (the Paragon and Dervish). Apart from that, Nightfall also includes hero NPCs and many Nightfall-only skills, so on the whole it is a very good package for a newbie.
Take note - each campaign has a very different feel to it. Prophecies and Eye of the North is European themed, Factions is Asian themed and Nightfall is African themed.

Prophecies is ALWAYS the best start for GW. It tells you the proper backstory and introduces you to all aspect of gameplay - and is long and bloody fun.
If you get other campaigns - then chances are you will not "enjoy" them as much since you wont have a clue WHY you are doing what you are doing and will probably think they are just mindless missions. Bloody fun nonetheless.
Nightfall is a mixed beast. It has its claws in all 3 chapters, and is a story in itself.
Best to play GW in the order they released them chapters - so you progress just as the story does. Your character will not be "gimped" in any way by the lack of all chapters installed. All cross-chapter items are freely available in the usual trade cities.
