There was this big update that wasn't tested much or at all? Well, it disfigured everyones faces due to some bug. My character had these protruding verts sticking out of his face. It was really horrifying to see everyone like that. I don't think it was even fixed for a month- that was scary too.
The first time I was asked to be a Tank for a group in EQ2. New to MMO's at the time, I remember my hands shaking when I clicked on the OK to accept the group message. I remember saying to myself, OMG I'm going to get everyone killed...oh why did I choose to become a tank, I could have been a healer, stayed in the back of the group, but noooooo, a Tank, right in the front line of the fight. Everyone depending on me to take the hits. If I die, they all die.
Maybe I can just hit the power button and tell everyone my pc died. I was horrified. Those were the days.
Now that was my most scary thing I can remember happening to me, but to answer your question of the most scary thing I've seen in an MMO..... that would be a dwarf running away from Giant. It just looked so scary. Poor dwarf.

I was outside dark elf village, there was a large field when you left the village on the right hand side where there were some walking mushroom things and these goblins, and I was hunting there.
Suddenly I see a character run over the hills and following him is this hook horror. I was going to help but realized I knew nothing about this thing and he had a good lead. Funny thing is that it didn't stop, it followed him right down the road to the village.
and unlike Wow where the guards attack mobs, I'm sure the guards at dark elf village did nothing. ; )
To this day I wonder if it took him down.

I was playing my first month of my first MMO, Anarchy Online, and as I tried to run into Old Athens, I fell through the ramp. For some reason, I ended up glitched. I couldn't get into the city, and there were no mobs and no people anymore. I was just running around in this barren wasteland. For some reason, it really creeped me out.
Edited to add: Okay, it still doesn't creep me out as much as people who spend hundreds of dollars every month in their game's cash shop. Nothing is creepier than that.
I am convinced there is only one way to eliminate these grave evils, namely through the establishment of a socialist economy, accompanied by an educational system which would be oriented toward social goals.
Venturing out into the Commonlands in EQ1 for the very first time, trying to find a wisp to kill so I could get a lightstone. (Yeah, there are bigger and scarier things in EQ1, but it was DARK, darn it!)

Well, heard, not seen (appropriate, considering how high quality the sound is in FFXI). FFXI has a creepy haunted dungeon called Gusgen Mines. It's scary enough to begin with (in addition to the actual undead monsters, there's ghostly images of dead people--including children--floating through area, appearing, disappearing...). There's also six ghost-type boss monsters (Notorious Monsters in FFXI parlance) that spawn on semi-random timers. When one spawns, a LOUD, shrieking siren goes off that can be heard throughout Gusgen and the nearby parts of adjoining Oldton Movalpolos. The first time I heard that, I must have jumped three feet. It can still make me jump a little.

I sometimes question the validity of community from all the seriously anti-social behavior I have seen in this genre. But no matter how bad I have seen people behave in MMO's it is nothing compared to the abomination of X-Box Live. That just makes you question our species right to exist.
Fallen Earth in beta has some bugs (as to be expected). Anyway near one of the starter towns there was this underground lab area where you get sent for a few quests. I get in the lab easily, as it was totally deserted, grab my item, and prepare to leave. Now I'm in first person view (the game plays a bit like an FPS), and I turn around to leave the room I'm in and almost fall off my damn chair from the shock of suddenly seeing about 20 zombies shambling through the doorway. Yeah, they ALL spawned on me. I only have a BB gun with like 8 rounds per clip. I killed maybe 2 before I went down in a heap. Now the bugs are ironed out and you have to fight the zombies to actually get into the room.
yeah its the people spending all their time in it especially having been an Everquest player. I warned this person in WoW when they were new to the game- don't get yourself too sucked in or whatever. Anyway anytime I logged in and resubscribed they were there playing. But people make and learn from their mistakes so.
Seriously, there are some people who are really messed up with severe mental issues who play MMOs. I'm talking about people with sociopathic tendencies, severe mood disorders, etc.
I have been in guilds with people who act perfectly friendly one day, and then the next they're verbally assaulting everyone and trying to destroy the community they're part of. Others, and some of the same, who act friendly to you and others, and then talk behind everyone's back to intentionally stir up drama. It's as if some of these people make a game of causing as much drama as possible. I have witnessed certain players virtually stalk other players, and harass them, some even going so far as trying to track them down in real life for whatever reason, be it obsessive desire, or hate for said player.
I would cast my vote for this as well. Having played a number of MMOs, been in various guilds, run a few guilds, and no issues jumping into PUGs, I've run into a number of very odd people...some of which become rather frightening (essentially getting to stalking). I've seen guilds pulled apart because of nonsense drama on the part of these people who thought playing the drama games was cool.

Sure there were moment in EQ when i saw a massive train heading my way, and my heart sank, but that inspired anger, not fear.

The scariest shit i've seen in a game was in Requiem, you know, when the in-game time hits midnight..... it's dark... a thick foggy mist rolls in... and then... the glowing eyes pierce through the mist, red, angry, unforgiving eyes... of a Nightmare spawn... their aggro range is HUGE, it screams, and takes off chasing, you - But.. your flanked by a cliff.. no choice but to fight this ugly psycho monster chasing you, then the battle ensues, you stand victorious, bleeding, near death. You smile because you survived the encounter... just as you sit to rest, you see 5 more sets of angry glowing eyes followed by shrilled screams.
But that game was a Horror-MMO, so it was expected, lol.

I can totally relate, except for me....I WAS the healer, and trust me....I was every single bit as TERRIFIED. Of course, you know as well as I do that everything always somehow ends up being either the healer's fault...or the tank's fault. There is no way in hell it's the fault of that pyroblast-type-of-spell spamming wizard, lock, mage, etc....uh uhh...noooo way it has anything to do with them.
