As others have mentioned, the production costs of a full fledged MMO including its world environment, content, classes, abilities, gear, etc, is all really damn high. Millions of dollars high. Investors put a lot of pressure on getting a return for the production and in most common terms, that's Box sales taking care of the initial negative balance that's been run up.
League of Legends has become an inarguable success based off of a free model video game however. I willingly predict we'll see more MMO's head the free to play route with a cash shop for shortcuts over a monthly subscription in our future. The production cost is the main hurdle being faced.

After reading this thread, it made me realize that RAGNAROK ONLINE PH ( that was 2003 onwards ) is still the best MMORPG that I've played so far.
