Sure, there probably will be costumes and other cosmetic items for character in the cash shop, but it undoubtedly will take some time for the devs to add enough cosmetic items to create a visually unique character.

As a general rule, Diablo style ARPGs (TQ, Sacred, DS and D3) were never much on the aesthetic customization that you are referring to. This will change for the first time with PoE's open beta cash shop.

Generally in an action RPG I prefer my characters look to be based on the items the character is wearing.
So I would look different based on if I was wearing leather jerkins or full plate. I wouldn't look like I was wearing a pink tuxedo while wearing full plate, and I hope that isn't the kind of visual unique character customization you're seeking.
Also, in most of my action RPG play, I prefer character customization to be more about customizing its abilities, stats, and features, rather than dress up.
