There were a few soundtrack threads before
but there's no such thing as "best" in music, music is all about taste and is different with everybody...
For me, among mmo's it's Knut's two AoC soundtracks (here's a live performance of The Dreaming: http://youtu.be/-g5KYv2oqXk) and Chance Thomas's LotRO ones, with SoA at the first place.
Outside mmo's, I listen VC's radios a lot, mostly V-Rock, same with GTA IV's Vladivostok FM
Doom and DoomII had also cool soundtracks back in the day, and I have a truckload of C64 collections (and a few Press Play on Tape versions) as well.

I also agree with Vanguard being right up there, as for a non mmo then i'm going to go with Gothic 3.
