Well heck, if we're wishing for the impossible we might as well go all the way.
A competition game with no cheating would require developers who are ready, willing, and able to banhammer the holy bejeezus out of exploiters from the very beginning -- something that should have been done all the way back in UO but wasn't, giving rise to a group of people who realize that cheating is often the very best way to 'get ahead'. This is what drove off people who would have liked pvp in the first place, setting into motion the events that we see today. But a large portion of pvp players are happy to admit that they'll do anything they can to ruin the other guy's gameplay still to this day because its 'fun' to them. And until the pvp community and/or developers can and will do something about those rejects, there is no pve/pvp solution that -can- present itself. Instead, any game that tries to blend the two overly much will literally be hosting the seeds of its own demise -- how else can you describe having a group of players who openly state that their goal is to run off other players?

For me the kill or die repeat a billion times to get some item ..yeah no challenge there at all. As we see in so many new F2P mmos anyone can do it.
Like "hey we can make a game were you just play on this tiny map and kill each other respawn and OMG people will PAY US to to do it." Then people are like WOW my gun has this new tint to it.. OOH I want it..
The "once you die its over" you find most never want to play that now thats the fun ones.

World of Darkness by CCP will be an open world sandbox with full pvp by their press release statements.
Dare I mention DayZ which sold over 1 million units in the first month of an alpha that they said not to buy. An open world MMO with full open pvp and permadeath and also it is still in the top 5 in steam sales. I will say that their number of players per server is still undecided at this point.
I could mention the large list of mostly independently financed development studios. Some of them will end up taking home the prize because the AAA studios and publishers cannot innovate with the exception being Sony. Sony has done this because they were getting into the situation where they had a stable full of shit games that were aging and they rest garbage and knew they were dying.

It sold over a million by the end of January this year and has been number 1 or in the top 5 since then so I can only imagine how many copies they have sold by now.

I am not at all convinced though these are early sales let's see 6 months to a year if they retain the numbers or grow then we can talk about trends and changes. Warhammer had amazing sales and so did SWTOR .Don't mean a hill of beans though a year or two down the road.
