I never have this problem. If I am learning how to do a dungeon then I look for a PUG advertising "anybody welcome" or "we're just learning so be patient" and that sort of thing. If I am wanting to do speed farming runs, then I learn how to do them first, then join the appropriate PUG.
Some people can't understand that type of concept though, and that is why we end up needing the kick option. It's unfortunate to say, but there are a ton of people in mmo gaming that can't understand basic concepts like "read the words on your screen" or "don't stand in the fire" These people have as much of a right to enjoy a button mashing funfest as the more capable people's right to enjoy a challenging trial of expertise.
The problem often lies in the fact that the ignorant have difficulty in recognizing when some things might actually be to their benefit. I would openly support of measure of qualification to be imposed on mmorpg games. Kind of a "you have to be this tall to get on the ride"

You have to really underperform and get a group wiped multiple times to get kicked from a PUG in my experience. Apart from the 0.5% of the time where players kick you for some unavoidably arbitrary reason (which can include them mistakenly believing you wiped them when it was another reason.)
Playing Vermintide currently (awesomely fun L4D-in-Warhammer-Fantasy co-op game) and have yet to see a single player kicked, despite several instances where a player was clearly braindead. :P
Generally In games I have no problem pugging and love to pug as it creates interesting dynamic challenges.

I've seen people kicked before dungeons even started or, if that's not feasible, people refuse to move until the "offending player" drops group. I wouldn't say several times, but multiple times.
Or, as I stated happened to me once, healers getting kicked because the dps couldn't kill the boss. After like 10 minutes in the boss fight of a level 15~ dungeon, we were just overwhelmed.
The irony is I could see they sucked and I was *sticking with them*, because I was trying to be the nice guy.
(Hint: If your dps takes >10 minutes on a boss in a newbie dungeon, something is very very wrong.)

Sad, but true. Online communities are getting worse and we should not see that improve anytime soon. It's why I no longer play MMOs. Single player RPGs ect for me. Much more fun without all the immaturity and elitist asshats.
Just play MMOs as single player games .. no more elitist asshats if you don't join a group or talk to anyone.

On both sides of that there are people that are doing somethign wrong and won't listen to advice, or get adversarial, who deserve to get kicked. And the real a-holes who kick people at the drop of a hat. Preventing this should be up to the game designer. Put options for Newbie OK/Scenic/Speed runs, so that you can play with like minded individuals. The games pop has to support that though, which is part of the problem.

Wrong is one way of putting it. I think PUG are the least skilled group and looking for something other than the least skilled is wrong. As in 2 + 2 = 5 type of wrong. If you want higher skilled play, you have options:
1) Guild 2) Find a like-minded group of friends in the game.

LOL I remember one time in SWTOR a healer was screaming at the tank over something stupid. He then tried to get the tank kicked. I ended up suggesting that the healer get kicked then used my companion healer to finish the event. Was good times. Lots of LOLs too.

The problem is already solved .. what is the problem with kicking people? Or quitting a group?
You have no obligations to play with any strangers. No stranger has the obligation to play with you. If anyone does not like it for any reason, kick or quit. What is the problem?

That is why independent loot (like WoW LFR, or D3) is better ... each person roll his/her own loot. No ninjaing is possible.

and some devs have taken it away ... it should not be a problem because technically it is very easy to make loot independent across players.
