It's an odd question... Unless I've missed something, PvE and death penalty are like fire and ice since years now. Maybe Wizardry was the last one trying something harsh, but it went down fast (and not just because of their penalty, might I add).
Check AoC, I think it still has the penalty it launched with 9 years ago, there were no revamps on it - but even that is just moderate compared to the "old" games' penalties.
I know none in games I have played. There was penalty of some debuffs in Istaria: if you are defeated, you get your skills deduced by 20%. Can be removed by eating foods in tavern (really lots of them). More death opints, more debuffs.
BDO has xp loss upon death, and a chance to break your gems. The first can be mitigated with items, the second, not so much. Painful once you start using the expensive gems, or the ones that can't be replaced due to being event rewards.
