I guess they had targeted the boss, ready to fire once we started and then hit some sort of attack by accident.
And then hilarity ensued. I think we got it together and were able to prevail.

I've done it, by accident of course. Really it was me accidentally starting a boss fight before everyone was ready by hitting an attack hotkey and almost wiping the group.
This was me in literally every single group encounter during the early months of LotRO: Shadows of Angmar. I was the Hunter no one wanted to group with.
I got better, though.

Plane of Fear break-in was far worse, IMO. Both could become a cluster fuck depending on circumstances and how bad the lag was that night
I think they changed Hate because initially it was worse. I used to log in and almost immediately get tells asking for ports to Hate. So I always got the impression that we were not the only ones having problems with Hate.
Sky was the other one and @Amathe was right about the noise level there. It was hilarious.

Hate was such a cluster. Port in, stand still, wait for everyone to get here then start pulling.
Fear was sending in the sacrificial druid/monk/necro to see if the mobs were where they were supposed to be before giving the ok for the raid to port in. Then run down the hill, take a left and hide (if I remember right).
I remember being a scout in there the first time. It's genuinely scary being the only person in a raid zone for the first time, knowing if you die someone will have to recover your corpse/possessions.
"LEROOOOOOOOOOY JENKIIIIIIIIIINS!"
I lasted about 5 seconds while the rest of the team was watching me. After teleporting me back to the group and rezzing me, the leader says, "OK, are you ready to listen to the PLAN now?"
The world is going to the dogs, which is just how I planned it!
Having played Everquest since I was 11 I have seen many real moments of people doing stupid stuff. One thing I remember the most from Everquest was during a raid we had to keep invis up so that we could move around without getting aggro from everything. Someone was not paying attention and had Spirit of Wolf on which makes you run faster. He didn't notice he was not invis any more and actually pulled nearly half the area. No one really noticed this either and when we stopped in what we thought was a safe area to refresh the invis we all turned around. What we saw was something like 30 mobs chasing 1 guy and we just laughed and was like well that was fun. Yeah, we all died laughing because it was actually pretty funny.
I have never been rude enough to try and kill my whole group for the lawls.
