Ultima online was launched in 1997 and is one of the greatest mmorpg games of all time, Runescape is another with not so nice graphics and still stands the test of time.
If you only look at graphics your not a gamer your just a complainer and to be honest this game is actually pretty good for a clan pvp orientated game.

The game is from 2016 and it has a steady population of around 300-400 players on Steam, I don't know how many play outside of Steam. I would personally never buy it because of the focus on PvP, but I won a free copy a few years ago and gave it a try and it confirmed my views that I would not buy such a game as I did not last more than 16 hours in it. There is almost nothing to do outside of preparing yourself for PvP.
But regardless of whether the game fits mine or your needs, the game has some kind of a loyal playerbase, which is probably good enough for the developers.

From what I played, PvE is minimal.
95% of their development resources went into PvP.
What I experienced outside of PvP is mostly very basic quests about fighting some mobs or gathering/crafting something. Basically to teach you combat and crafting, which is just to usher you toward gearing yourself up and getting prepared for the PvP.
In the trailer you can see a group of players exploring something like a dungeon, fighting something like a giant and what looks like skeletons. The developers explain that the PvP is similar to Albion Online where you have the central zone with full loot PvP and then the zone around the central zone are something like "half-PvP" and the zone outside of that is non-PvP. But I explicitly remember that sentence "You can avoid PvP, but only to a certain extent."
I've had my free copy since 2019 and since then I've been asking for more PvE content, but every time me or someone else did that, the players would pitch in and say that's probably not gonna happen and that we should probably find another game, developers have been more ambiguous in that they are not saying outright they won't focus on PvE, but "they are looking into ways of expanding non-PvP activities".
So I'm not sure if they added anything to do between those 3+ years. The most prominent change I saw was them revamping the UI. Also since I had the game, the performance was not good, I still haven't tried their 1.0 release, but will probably do in the following days and see what's up.
