As for me I have a handful of names I reuse depending on genre that are names I've used before all the way back to my table top D&D days. It's either due to nostalgia or my lack of creativity. Maybe both!
Anstalt means "asylum" (as in insane asylum) in German....or so im told, i dont speak German! I feel like it suits melee focused characters well, especially if I can have a beserker spec. Plus, I've never had the name already be taken. I tend to use it on Dwarven characters.
Spit is my favourite orc name, and with orcs being my favourite fantasy race I tend to use it a lot. I wanted a name that was both aggressive and a bit disgusting, so I feel like I nailed it with this one!
Scab is similar to Spit, but less aggressive and more disgusting. I use it for goblins, or alt-orcs.
Ragnar is just because im a big vikings fan. This is usually my go-to name for human males, but in MMOs its usually taken.
Jennifern is because Jennifer is my favourite female name, and with the "n" on the end it felt a bit more homely, a bit more country. I used this name on my main in LotRO for 5 years and became very attached to it.

I use Mendel a lot because it could be many different languages from Arabic to Polish. I also use Walvis, Chesterton, and Palmerdale a lot these days, they sound like characters. I've also used obscure words, like Quend, Uvvu, and Zyzzyva.

My favorite name came from Skyrim: Riik Stormdancer.
"Riik" means "Gale" in Dragon Tongue.
"Stormdancer" was cool (I thought) Nordic name.
My "idea" was to create a Dragonborn Nord and run around using my Storm Call shout with dual blades. Call a storm, start "dancing about" lopping off heads while thunder rolls and lightning strikes my foes. This worked until my first involving city guards. Those strikes do not know friend from foe. lol
Another name I use frequently in gaming is Menolly. I took this from Anne MacCaffrey's Dragon Riders of Pern books (The Harper Hall series). In her book about the Harper's Hall, we get to meet and follow Menolly from her fishing hold to being discovered for the Harper Hall. I love that story!
I've tried using "Al B. Quirky" a few times, but that never stuck with me

Lara Croft is a beloved character in the gaming world for many reasons! Not only is she a skilled adventurer who can take down any challenge, but she also has a killer fashion sense and can rock that signature braid like nobody's business. Plus, who wouldn't want to explore ancient tombs and uncover lost treasures? Lara Croft is the epitome of badassery, and that's why so many gamers love her!
I usually name my character Krugos; I don't really like my real name, so I never use it. If playing with a female character or several characters, I have a few other names that I use often.

Ok. Whom should I use? Let's use my SWTOR Sith Warrior. I wanted an example of a slow burn, of internal, everburning, controlled rage. So, my thoughts naturally went to Embers and such concepts. But that's a silly, non Sith name. So I was like ... Lord Ember ... ? Emberspear. That sounds Sith Pureblood enough. Especially when I imagined him coming more from the Hunter Caste.
But! That isn't enough, is it? Yes, let's be 100% original. What can we do? Well, this is a 1000s old species(even at time of SWTOR), no? Do they have their own language? Most certainly: https://starwars.fandom.com/wiki/Sith_(language) . And so it is:
Lord Chwuq'Sutta.
(There is a chance I should invert the order of those words, but this just sounds ... right and as such, I don't care. Besides, should anyone decide to copy me, I can easily check stuff, with that ace up my sleeve)
(I generally invest great effort into merging who my character is{class, looks, name etc} with what my vision for it is)

Well the famous tv show "Die Anstalt" or "Neues aus der Anstalt" (news from the asylum) is indeed an insane asylum ... as the backdrop for political comedy.

In single player games I usually pick names like:

Or, in short, I try to find pretty names.
My multiplayer names are easy to explain: I always use a new one that I never used before. Pretty much like forum names. The difference is, with multiplayer names I try to come up with names that actually fit the scenario. Like when I was playing Qaliathari in Vanguard, I looked up arabic names for my characters. For the record, it was actually pretty hard to find names that I actually liked, and that was also still available.

I got the idea from Baldurs Gate, where "Victoria" turned into "Viconia".
