If I were limited to only one, I would say non-consentual PVP/griefing. If you turn the game I am playing into a gankfest, I will leave so fast I won't hear the echo of the door slamming.
Quest hubs and on rails tutorials do seem to off put a lot of people. Which is why I don't like the way Runescape currently has it set up. You start on an island and basically are taught how to move, use items and fight. which is itself something of a "My first Quest" And when you leave that island you see a lot of NPC's with icons over there heads. They don't have quests for you though. They are skill tutors.
The game has 26 skills and the devs realized trying to cram them all into on tutorial was front loading the entry level too much. So, a few takes at a new tutorial system later they have come out with this take it or leave it as you want to learn it system. The only problem is that when you first start out. You are in a guided tutorial mode. This is easily checked off so you can explore openly and encounter content simply as you happen to. But, that isn't made obvious.
So, when someone first enters runescape. it kind of has that feel. even though through out most of a runescape career you likely started a quest because you saw an npc in the middle of know where up to something peculiar and had your interest peaked. So, there are few games where it sounds like a duck, and walks like a duck. But, then you find out it isn't a duck. The sad thing is a lot of us are jaded to a greater or lesser extent. And, we hear "quack" and are looking for the door.
That is the sad thing about all of our aversions. We may have each picked out a different thing. Something that our own experiences has taught us will save us a bad experience. And, that is conditioned by all the bad experiences we have had. Most of the time that aversion may be a perfectly indicative of our expectations...But, once in a while. think of what we may have missed out on because of them. Of course runescape is a very polarizing game. People tend to love it or hate it outright. But, I have played it for 14 years. And, had I these aversions when i was fresh to MMORPG gaming. I would have dropped it shortly after login.
Free to play. I dont want to think about spending real money while playing. I am more than happy to pay $15 a month. It is a bargin compared to buying PS4 games at $60 and only lasting 2 weeks.

I cannot stand the new trend in mmo moving to single bar for abilities. I know a lot of people dislike SWTOR and the need to have two full ability bars, but I love it. It allows for some diversity in combat instead of 1,1,2,1,3,1,1,4 and repeat. Every single-bar game I have foolishly purchased or downloaded was uninstalled within a month, two at the very most. Just does not have the longevity for me once I hit the "every fight feels EXACTLY the same" this happens about half way to cap in these games for me.
Great thread though, forced me to think of what I hated most. Now that I have pinpointed it I will just skip all "action" combat mmos in the future. Wow might have to be shelved if the trend of removing abilities continues, I don't play MOBAs.
What I mean by that is a MMO that I put my time and money in to and they change the game to something else from what it was. For example in Wow back in Vanilla and TBC they had a leveling game that had a good balance of not to hard and not to easy. When WOTLK launched they made the leveling game to easy and kept nerfing it to what we have today which is a joke.
There is many more features that turn me off but you only wanted me to list one.

Invisible walls.
I like FFXIV but i HATE the amount of useless invisible walls in that game. Specially in Limsa Lominsa where i want to just jump down to the water and take a shortcut to land, but no.... the "land" next to the city is a background image, you cant reach it without using the city gate and a loading screen.
Auto-follow. No, seriously. I can't count the amount of times I have been in PUGs from the LFG tool, and have had someone go on Auto-follow throughout the entire instance!! You can't even get them killed in WoW anymore because of the ease of the instances.
Oh.....and the LFG tool.....but, that's another story.

I also like to have a lot of abilities to use, makes the combat more fun to have options instead of just having few. Even the very rarely used abilities are nice, the character feels more deep. Also sometimes it's just nice to fool around with some abilities.
I think that single hotbar with only few active abilities is one reason that hastens my boredom in mmorpgs, the character feels way too simple, even if you can respec or change hotbars like in ESO, it's a big downside.
Non-consensual PvP is number one. If the game has it, it's not even on the radar.
After that is a system of game control that's not remappable. I play with few different control quirks: If you think I am going to switch because you think your game is super special......well, your game had better be super special.
Poorly designed/written game world. While not always obvious immediately, this failing leads to uninstalling as well.
If you are holding out for the perfect game, the only game you play will be the waiting one.
1) it has to be a world I enjoy being in. Before the gameplay, I have to enjoy just being in the world. And things have to make sense in that world. For example, I got into the SWTOR closed beta, and quit within an hour. The world seemed dead to me, no animals, no wind in the trees, and too closed in. Plus, I got into a corner where if I tried to get out, I would be attacked. I know! Send my minion out to do crafting. Total immersion breaker. How can he run past the mobs and I can't? Dead world? No thanks.
2) Console restrictions. The latest fad is to just give enough action spots that will fit on a controller. So what if I've learned 25 abilities? I still can only use 5 at a time in some games, like ESO. And the inventory system? A complete butchery trying to make it work for the console players, while making it ridiculous to PC players. The inventory system in ESO is pathetic, worse than any MMO I will play, and just as bad as the worst console ports.
Toss up between achievements and forcing people to visit cash shops in game windows.
How the fuck does achievements ruin an mmo for you? Like what?
Because they are annoying and I don't know of a single game where you can turn the notifications off.
I don't care that I've killed 10 boars, eaten 20 apples, or stepped in dragon poop.
I'm trying to play a game and don't need to be reminded that I'm doing things that have no value whatsoever. I'm already doing something that has no value - I'm playing a video game.
Inventory isn't my biggest pet peeve, but OP did make me miss my Crat in Anarchy Online. Throw your full backpacks in your bank and as a Crat you could access thousands of items anywhere in the game.
Of course I could never find anything, either.
My biggest pet peeve is forced grouping. Which generally means I avoid open world pvp games, too, as solo players almost always end up as ganker fodder.
I am convinced there is only one way to eliminate these grave evils, namely through the establishment of a socialist economy, accompanied by an educational system which would be oriented toward social goals.

Well 'ruin' is probably too strong, but I don't particularly care for games with emphasis on questing and pve raid progression.

These are my minimal requirement to even bother to think about some game:
3rd person view
mouse for moving (if WASD only I’m definitely NOT playing) and 1-0 keys for spells
not only single toolbar for abilities/spells (like Wildstar or TSW, where I stopped to play just for this sole reason)
And yes, ... love tab targeting.
