Can you believe this game once dominated the PnP RPG industry back in the 80's? It has 30 years of material and is still around today. It just turned out the guys who originally ran D&D (TSR) where better marketers. Once they sold to WotC (Hasbro), mainstream success/marketing was a given.
Sci-Fi Western? easy! set the game in the Firefly/Serenity world. That series had the 1st instance i ever saw (and was mightily impressed by) of Cattle Rustling using a Space Ship
Also.. set a game in the Farscape universe... hell, living ships! talk about the ultimate in Creature Handling
Both these setting woulds lend themselves so well to an MMORPG game
Anything as far away from reality as possible... think about it... IF the virtual world is real, then a 'quest' turns into a 'chore'...
NPC: Ok. I want you to harvest clothes from around the house and wash 5 loads in this machine. When you are done, come see me and I will give you a token you can use at the mall.

Setting: Earth now (20th - 21st century) Genre: Organized crime
The game would start out as sort of Grand Theft Auto online. You would start as a common street punk taking jobs from different gangs and committing petty crimes. You could then join a gang (either NPC or character-run) and work your way up the ranks. In the beginning, all the gangs would be run by NPCs, but players could take control of existing gangs or start their own, so eventually most gangs would be player run.
A player-run gang would basically be like guilds in other games. Additionally, you could hire NPC henchmen to help protect your base from raids. You or your (player) lieutenants could give orders to these henchmen. You could also assign jobs to your underlings or to unaffiliated players, which would add player-driven content.

Future... with mechs.
I'd love to see a well done mechwarrior MMO... but I'd settle for a mediocre Starsiege MMO at this point
